Upload Your Own Photos

To submit your digital photograph via email:

  1. Download Foxit Reader here.*
  2. Fill out the form and click File then Save As using your first initial lastname format.**
  3. Please put your full name in the Subject line of the email.
  4. Attach your digital photograph and consent form to an email addressed to SCHCWebPhotos@sheridan.hpmin.com

To submit your photograph via USPS:

  1. Print off and complete the consent form click here and complete all required fields
  2. Please write your name on the back of the photograph
  3. If you would like your photograph returned via USPS, please provide a self-addressed stamped envelope along with your photo submission

 

* Foxit Reader or Adobe Acrobat is required to save the data to the pdf. Adobe Reader will not work.

** Please name the file in the following format: FLast where F is the first initial of your first name and Last is your last name
